I grant Grace Church and its employees or agents permission to take photographs, record audio, or capture video footage of myself and/or my child for use in its various media productions. I recognize that this use includes the current and future display, distribution, transmission, or publication of captured material of myself and/or my child for newsletters, promotional videos, digital website images, and other media productions. I recognize that mine and/or my child’s name and identity may be revealed in the production of this material, and I waive any right to financial compensation or other forms of payment for mine and/or my child’s participation in this production.
